College of Music Release Form

For good and valuable consideration, the receipt of which from Florida State University (FSU) is acknowledged, I hereby grant to FSU and to its employees and agents the right to photograph me and use my picture, silhouette, and other reproductions of my physical likeness (as the same may appear in any still camera photograph, motion picture film, videotape recording, electronic or Internet formats of any kind) [“photos”] in and in connection to recording sessions or performances, including live streaming, when I am performing as a soloist or a member of any FSU College of Music ensemble during the 2020-2021 academic year. Furthermore, I give and assign FSU the perpetual royalty-free right and license to record, mechanically reproduce, and distribute my voice and all instrumental, musical, or other sounds produced by me during those same recording sessions or live streamed events. I grant FSU the authority to distribute those recordings in any format or manner now known or to be known in the future, including photos (whether as originally recorded or as modified after further postproduction work), and synchronization rights, privately, publicly, or commercially, on FSU’s own, by assignment, joint venture or other arrangement with third parties for the financial, public relations, or reputational benefit of FSU, the College of Music, and those partners as deemed appropriate in the sole discretion of FSU and the College of Music in the future. Should FSU or the College of Music distribute all or any portion of these recording or photos in any manner described above, I understand that I will receive no royalties for the recording or distribution, even if it is released commercially. I will nevertheless reserve the right to list my participation in that recording on my professional materials, both digitally and in print. I hereby certify that I have read the foregoing and fully understand its meaning, am over the age of 18 years and fully competent to sign this agreement and agree to its content.
